Analyze the theory and explain either how it could be applied to a clinical setting in which you have worked or how it is currently used in the clinical setting. (Analyze means to look at both the good and bad points and compare them) For example, if you chose a theory based on holistic nursing you need to analyze how this applies in your current clinical practice. You MUST give examples of how it is applied. You may use the word “I” in this assignment.

Example of applying a theory to current nursing practice below
Although over 50 years old, Abdellah’s theories are still evident in the workplace today. For example, Abdellah focused on four major concepts – nursing, the person, health, society and the environment (Reference). This is a holistic approach which is endorsed by many Australian hospitals and the World Health Organization (WHO) (reference). The patient is looked at as a “whole” person and as nurses today we not only fix up the “health” issue but identify and assist with their social environment and any psychological issues. An example of this in my current workplace is how we begin discharge planning from day one of entering hospital. A recent patient I cared for was very stressed that their husband would be unable to cook for himself whilst she was in hospital. The stress was affecting her health and she could not rest. Gore (2010) identified that stress affects all body systems and can cause a delay in healing. I organized for “meals on wheels” to be delivered to her husband until the wife’s return.

Reference list and include in-text references in APA format. (at least 3 credible references. CINAHL in the GCIT online data bases has some very good journal articles on nursing theorists.

Reflection - Written Assignment

Reflection –This assessment is asking you to question your own nursing practice in a critical way, to enable you to take action and learn through reflection. Nursing is unique in that although knowledge drives our practice we are often exposed to situations where empirical knowledge is not enough and we must also rely on intuitive knowledge. There is no right or wrong answer when reflecting – just the ability to internally examine and explore an issue of concern, triggered by an experience that results in you looking at the event with a changed perspective.

Task: Choose an Event that has Impacted on your Nursing Career Whilst on Clinical Placement or at a Health Facility where you Work.

Document your reflections on what happened and support your argument with current research:  Write your assignment under the following headings.

1. Description: Describe what actually happened: Where, When, What (What essential factors contributed to this experience), Who was involved, why.
Example: John Barnes (fictitious name) was a 52 year old man admitted to my ward with an abdominal obstruction that could not be resolved. Over the period of two weeks he was on the ward many of the nurses found him to be rude and difficult. During handover nurses complained about his behavior and no one wanted to look after him. On the morning of the event he was put in my care, which I was not happy about. On that morning he rang his call button several times between 7am and 9am.  After morning tea he began ringing it again and I decided to ignore it.  Etc.  He died shortly after a code blue being called. Etc

2.Reflection: What were you thinking and feeling at the time.
For example: What was I trying to achieve: Why did I intervene as I did? What were the consequences of my actions for: Myself, the patient/family, peers/colleagues?. How did I feel about this experience when it was happening? How did the patient feel? How do I know what the patient felt?

Example: On reflection I realized that I had ignored the call bell because I didn’t really like John and I was annoyed that he kept ringing his call bell. I remember as it was ringing I was thinking “Go away you miserable old man”. If I had known he was dying and calling for help the situation would have been different etc.

3. Evaluation:Analyze the situation
What was good and bad about the experience
For example: What was done well by you or others
Example: Looking back on the situation I felt that I really wasn’t listening well at the time of the code and that’s why the mistake happened. I was feeling so guilty about not answering his bell. Jones (2010) investigated code blues and found that a lack of communication between doctors and nurses was a strong factor for nurse errors.

What could have been done better by you or others
Example: 
If the doctor had realised that I was only a student I think things may have worked out better. Inexperience during codes (Davis, 2006) is a major contributor to errors. If I had only spoken up and not been so scared this incident perhaps would not have occurred and John would be alive today. In a recent study by Smith (2011) it was discovered that fear is a big contributor to communication problems. If I am really honest with myself, I think the incident may not have occurred if I had also listened and answered the call bell when John rang. Interestingly, Barker (2009) maintains that when we reflect on situations it opens us up to a deeper way of thinking and also allows us to come to a resolution. Reflection according to Jones (2010) is cleansing and allows for personal growth.

4.Learning: If it happened again what would you do?
What have you learnt from this event? Has has this experience changed your ways of knowing? What personal changes have occurred? Has it changed your morals/ethics or nursing practice?
